Admissions Coordinator
Position: Health Care Admissions Coordinator & Resident Services Liaison
Reports to: Administrator & Executive Director
Salary Class: Exempt

Minimum and Desired Qualifications:
- Written and verbal communication skills
- Ability to explain complex information to residents and family members
- Proficient use of Word and Excel
- Knowledge of payer sources including Medicaid, Medicare, Commercial Insurances.
- Ability to determine prospective patients’ insurance benefits and eligibility as well as obtain timely insurance pre-certifications
- Ability to work under fast paced & stressful conditions
- Minimum of one year experience in admission coordinator role at a long term care/rehab facility is desired.
- Knowledge of, and ability to connect clients and or their social support systems to services and programs for seniors
- Ability to work with an interdisciplinary team of medical professionals, including physicians, nurses, social workers and nutritionists.
Position Description: The primary duties of this individual in the role of Health Care Admissions will be facilitation of the admission and readmission of patients to the skilled care unit from hospital and community referral sources. In the role of Resident Services Liaison, the primary duties will be the identification of client needs beyond services provided by Treyton Oak Towers staff members. These may include physician or physician specialist referrals, referrals for adaptive equipment, referrals for legal assistance and other needs as identified by the Liaison.
Duties and/or Responsibilities:
- Facilitate communication between community liaison(s) and clinical staff at facility regarding all prospective admissions and readmissions.
- Provide inquiring residents and family/representatives with informational tours about facility services; assess prospective patients’ needs within scope of expertise.
- Manage telephone inquiries about care, services and availability at this facility.
- Verify eligibility and benefits for prospective patients for multiple payer sources, including Medicaid, Medicare and Medicare replacement plans and other commercial payer types.
- Obtain precertification of patient benefits as necessary for prospective patients.
- Complete admission paper work with each new patient/resident and/or their responsible party.
- Inspect inventory of available rooms and address needs for cleanliness, repairs and equipment as indicated.
- Initiate the patient record in the facility’s electronic health care record; notify team members of pending admissions, payer source, MD, and other essential information.
- Participate in the Treyton Oak Tower Continuum of Care weekly meeting for the purpose of both receiving and sharing information about residents within the community related to care needed.
- Work with PC Unit Director and the Resident Services Director for the identification of residents in need of referral for services or programs.
- Communicate with residents and their family members regarding resident needs; facilitate the residents’ enrollment in services or programs as indicated.
- Educate Treyton Oak Tower’s Independent Living residents about services and programs for seniors through formal and informal methods.
- Is responsible for any and all personal belongings brought into the building.
- All other duties as assigned.
