Path Case Manager
LifeHealth Services is seeking a Path Case Manager
Job Description:
The PATH Case Manager will provide assistance in obtaining and coordinating services for eligible homeless individuals. In addition, the PATH Case Manager will provide community outreach, internal & external staff training, and case management services in the ten county service area. The PATH Case Manager will also ensure data and program compliancy with all state and federal grant guidelines.
Requirements:
This position requires a Bachelor's Degree, with one year of housing related knowledge preferred. Must have fundamental knowledge of Microsoft Office programs; possess excellent written and verbal communication skills, efficient organizational skills, detail oriented and able to work in a fast paced environment while managing multiple tasks independently.
Travel is required in LifeSkills ten county service area.
Applicant must also have a valid driver's license and reliable transportation with current insurance.
